Health Link Home Health Agency is seeking a Per Diem Hospice Nurse (RN) to provide care in our patient's homes throughout East Bay in the San Francisco Bay Area. As a Registered Nurse, you will evaluate the patient's overall health. Your goal will be to improve the quality of life for your patients. All Nursing will be done in accordance with each patient's individual plan of care.

For over 15 years, we continue to serve as a trusted and local in-home health care provider with expert knowledge in rehabilitative care, elderly health care, post-acute and surgical care.

We are looking for compassionate clinicians to join our amazing team of healthcare professionals.

Job Summary:

Registered Nurse hospice to provide nursing care to terminally ill Hospice patients as needed. Help by providing assistance and understanding to the family in the home care situation and in the time of bereavement. To work as a member of the Hospice team in providing Hospice care.

Qualifications:

Registered Nurse Graduate from an accredited school of nursing

Currently licensed as a registered nurse to practice in the state of California

Hospice Nurse ability to work independently, make accurate and at times, quick judgments.

Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting.

Acceptance of and adaptability to different social, racial, cultural and religious modes.

Minimum 1 year of experience as a Hospice nurse (Registered Nurse)

Ability to travel within a 15 to 25 mile radius on a regular basis
