Job Description:
Job Description *This role is primarily remote with 1-2 days per
week of in person work at our Petaluma Clinic. Must reside in CA to
qualify for this role. Summary: The NP/PA Clinical Support Team
Provider will serve as an integral member of the Petaluma Health
Center’s primary care team and offer critical treatment plan follow
up and support for patients at Petaluma Health Center’s sites. The
Clinical Support Team Provider will ensure that patients,
particularly those at high risk or with complex care needs, have
access to timely and appropriate care between visits with their
primary care providers. Critical to the role will be the ability to
navigate the electronic health record (EHR) and manage the flow of
information to improve health outcomes for patients. Areas of
Responsibility: Team Support and Coverage: Provide support to teams
to ensure timely review of labs, diagnostic imaging, and documents
for patients between visits. Ensure patient-centered focus on
timely resulting of diagnostics ordered by primary care providers
or urgent care providers by working with the Clinical Support Team
to communicate these results and any follow-up needed subsequently.
Be available to primary care teams at all sites as backup for
urgent questions or review of results if primary care providers are
unavailable and patients or hospital providers need assistance for
coordination of care Be available to primary care teams at all
sites as backup for urgent medication review or refill Patient
Care: Provide patient visits via telehealth to review urgent or
timely diagnostic results Provide in person or telehealth urgent
care on a regular schedule or as needed Work with patients to
establish individual health goals and identify potential
problems/barriers. Assist in timely scheduling of medical
appointments to support patient health. Ensure that pertinent
diagnostics are ordered and appropriate therapies are implemented
according to scope of practice guidelines. Communicate with
patients so as to improve their understanding of their health care,
facilitate appropriate health maintenance and treatment seeking
behavior, and optimize their care outcomes. Function as an integral
part of the clinical staff through participation in Medical Staff
meetings and other PHC committees or workgroups Demonstrate
competent knowledge in delivering health care to patients through
the peer review process Maintain credentials by keeping all
required licenses current, including but not limited to California
Medical License, DEA licenses, CME hours, BLS certifications and
any other required licenses and/or certifications. Willingly
perform other duties within the scope of practice as assigned Abide
by all Joint Commission Ambulatory Care requirements including, but
not limited to, sensitivity to cultural diversity, patient care,
patients’ rights and ethical treatment, safety and security of
physical environments, emergency management, teamwork, respect for
others, participation in ongoing education and training,
communication and adherence to safety and quality programs,
compliance with National Patient Safety Goals, and licensure and
health screenings. Required Education and Experience: Completion of
a nationally accredited Nurse Practitioner or Physician Assistant
Program Two years’ experience as a licensed NP or PA provider
delivering primary care in a family practice setting, or graduation
from a one-year NP residency program Experience working in a NCQA
Patient-Centered Medical Home highly preferred Experience working
in a Joint Commission Accredited health care organization highly
preferred Desired Skills: Comfortable with use of Electronic
Medical Record Modern practices and principles of method disease
counseling Demonstrates knowledge of compliance issues within the
community clinic environment Strong interpersonal skills; ability
to be sensitive and communicate effectively with persons of various
social, cultural, economic and educational backgrounds Strong
organizational skills with ability to prioritize projects, work
relatively independently, manage multiple tasks, and meet deadlines
Strong written and verbal communication skills Ability to work
independently and as part of a team Good judgment, problem solving
and decision-making skills Licenses and Certifications: Current
State of California NP or PA license NP and PA must hold current
national board certification from an accepted certifying agency and
provide by first day of work. NP must hold a current and active RN
and NP and Furnishing license in California. NP must have completed
a BRN recognized nurse practitioner graduate master’s degree
program; PA must have completed an accredited physician assistant
master’s degree program Must obtain an active and/or transferable
DEA license to California and provide evidence by 90th day of
clinical practice. (It is the responsibility of the employee to
transfer their DEA license to their new place of employment if from
out of state, via online DEA portal and provide evidence of
transfer). Must maintain CME as required for certification
renewals. Must hold a current BLS Certification Candidate must be
able to successfully meet PHC’s credentialing and privileging
requirements Language Skills: Ability to effectively present
information and respond to questions and requests from patients,
co-workers, and others as necessary. Bilingual in English and
