Physical Therapist Location: Rancho Cordova, CA Agency: Thrive Staffing Pay: $1,898 to $2,092 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/17/2026 About the Position Job Title: PT (Physical Therapist), Rehab City/State: Rancho Cordova, California Shift: 8hr Days, 08:00:00-16:00:00, EOW Contract Length: 91 weeks Start Date: 03/17/2026 Job Description: Provides physical therapy services to patients/clients according to a written physician's plan of care. This includes assessing, evaluating, and updating therapeutic/rehabilitative/functional status, evaluating home environment, instructing patients/clients and families/caregivers, and reporting the patient's/client's reaction to treatment. Responsibilities: Assessing, evaluating, and updating therapeutic/rehabilitative/functional status. Evaluating home environment and assessing muscle strength, mobility, gait, and ROM. Instructing patients/clients and families/caregivers on therapeutic appliances and physical therapy goals. Reporting patient's/client's reaction to treatment to the physician. Initiating physical therapy program and supervising personnel and family/caregiver members in therapy phases. Training patient/client in the use of prosthetic devices. Supervising homemaker/Home Health Aides in rehabilitation regime. Identifying needs for other home health services and referring as necessary. Preparing and submitting clinical and progress summaries to the attending physician and health records. Providing physical therapy consultation to families/caregivers and inservice education for nursing agency personnel. Participating in peer consultation, staff competency assessment, and compliance with regulations and licensure requirements. Requirements: 2 years experience in a healthcare setting required; community/home health experience preferred. Home Health experience including OASIS required. BLS and NPI required; CA PT license needed. Current driver's license and proof of automobile insurance required for HH. Good oral and written communication skills; good organizational skills. EMR HomeCare Home Base (HCHB) experience preferred. Requirements Required for Onboarding BLS Core Mandatory Part III
