About the Position Job Title: Case Management Assistant Profession:
Case Management Specialty: Case Management Duration: 13 weeks
Shift: 8H Day Hours per Shift: 8 Experience: Minimum years of
experience required Description: This position will work directly
with the Case Management and Social Services team. The role
involves providing support services to assist with the timely
discharge and transfer of patients from the hospital. Clerical
