Health Worker (CHW) Reports To: Regional Program Manager
Department: PneumaCare Health & Wellness Position Summary: The
Community Health Worker (CHW) assesses and supports neighbors'
needs related to physical and mental health, substance use
disorders, oral health, palliative care, memory care,
trauma-informed care, social support, housing stability, vocational
assistance, and overall wellness. This role actively collaborates
with neighbors, their families, caregivers, community support
teams, and healthcare providers to coordinate comprehensive care,
promote independence, recovery, and resilience, and ensure
effective linkages to community-based resources. Essential Duties
and Responsibilities: Conduct comprehensive assessments to identify
neighbors' health and social needs. Develop personalized care
plans, coordinating and managing care within Enhanced Care
Management (ECM), Community Supports (CS), and CHW services.
Utilize evidence-based practices including Motivational
Interviewing, Stages of Change, Harm Reduction Techniques, and
Trauma-Informed Care principles. Facilitate communication and
collaboration among neighbors, caregivers, healthcare providers,
and community resources to ensure seamless service delivery.
Conduct outreach and engagement activities to effectively connect
neighbors with appropriate ECM or CS programs. Advocate for
neighbors, promoting empowerment and self-sufficiency while
maximizing their access to beneficial resources. Participate in
ongoing training programs, including but not limited to Adverse
Childhood Experiences (ACEs), Mental Health Care (MHC), and CHW
certification training. Obtain consent from neighbors to advocate
on their behalf with healthcare professionals and other service
providers. Drive MACC vehicle to pick up food from food bank for
office food pantry supply. Minimum Qualifications: An Associate's
degree in Public Administration, Business, Social Work, Psychology,
Marriage and Family Therapy, Emergency Management, or related
field; OR Two (2) years of professional experience in home health,
social services, or case management specifically serving low-income
populations; OR Any combination of education and experience that
provides the necessary knowledge, skills, and abilities to perform
the duties of the position. Desired Qualifications: Experience in
program administration, including strategy formulation,
team-building, and cross-departmental collaboration. Excellent
interpersonal and communication skills, both verbal and written.
Experience implementing health systems quality improvement
initiatives and using quantitative data to inform and drive change.
Basic knowledge of Medi-Cal programs and eligibility requirements
for public agency services. Demonstrated passion and commitment to
serving underserved populations. Additional Requirements:
Possession of a valid California Class C Driver License in good
standing, personal auto insurance, and insurability under the
organization's insurance carrier.
